Health Checks for New Local Patients
Between 2021 and 2022 LADMS were approached by the Lincolnshire ICB to assist local GP Surgeries in providing new patient health checks for patients who have recently migrated to England/Lincolnshire Coastal Area.
As part of this service, LADMS worked closely with SENCO, the Language line, the Home Office and the ICB to ensure care was provided to the newly registered patients.
A new patient health check allowed LADMS and other NHS HealthCare services that look after the patients to have an understanding of the patients:
- Demographics
- Observations, including Height, Weight, Blood Pressure (BP), BMI
- Medical history
- Social history
- Family History
- Current medications
- Current diagnosis
- If there were any tests required
- If there were any referrals required
